Referrals
A provider may determine that a patient should see a specialist for additional testing and/or evaluation. This may be to a specialist on staff at Student Life Student Health Services (SLSHS) or an outside provider.
Inbound Referrals
If a provider refers to a SLSHS specialty provider, an inbound referral will be created and the patient will be directed to Advice and Appointments to schedule.
Outbound Referrals
Non-Urgent
If a provider refers to an outside specialist, a patient will be asked to:
- Complete a referral scheduling form
- Provide a copy (both front and back) of an insurance card
The referral team will contact the patient within 2-3 business days with appointment information.
Urgent
An urgent referral needs to be seen within 48 hours and a patient will be asked to:
- Provide a copy (both front and back) of an insurance card
- Meet with the referral team who will schedule the appointment with the specialist
Self-Referral
A patient may prefer to see a specialist and make their own appointment. However, some insurance plans and/or specialists require a referral from another physician.
Patient Responsibilities
- Ensure each service and provider is covered by their insurance plan prior to making an appointment
- Pay all charges not covered by health insurance
- Contact the specialist to reschedule/cancel an appointment at least 24 hours prior to the appointment
