All signatures on forms must be signed manually.  Electronic signatures are not accepted.

All of the forms to the left can be completed online and printed.  When completing by hand, please use black ink.  This allows for a clearer image when scanned into your health record.

To submit a form:

  1. Complete the form.
  2. Proofread the form and ensure the information is correct.
  3. Save the form.  From the file menu, select SAVE AS.  Select a location on your computer or flash drive and click the SAVE button.
  4. If signature is required, print the form and add your signature.

Completed forms may be submitted:

  • Online via My BuckMD, document upload
  • By Fax, see About Us/Contact page for listing of fax numbers
  • In Person with the appropriate department
