Allergy Therapy Frequently Asked Questions
1. Can I schedule my first allergy shot appointment before submitting my paperwork and vials?
No. We must receive and confirm all required forms and allergy vials before your first appointment can be scheduled. This is to ensure patient safety and clinical readiness.
2. What documents and materials are required before I can schedule?
You must submit all completed forms from the Allergen Immunotherapy Instructions and Forms packet, along with your allergy vials, as prepared and shipped by your prescribing allergist. All required forms and instructions are available in the Allergen Immunotherapy Instructions and Forms document found on our website https://shs.osu.edu/services/allergy-therapy.
3. Can I schedule appointments in advance after my first visit?
Yes. Once your initial appointment is complete and all required materials are on file, we recommend that you schedule future allergy injection appointments in advance to stay on the prescibed schedule. You can schedule online via My BuckMD or call 614-292-4321.
4. Where can I find the required forms?

5. How long does it take to process my paperwork and vials once received?
Processing typically takes 2 business days, depending on clinical volume. You will be contacted via My BuckMD once everything is reviewed and you are cleared to schedule your first injection appointment.

7. Where should I send/deliver completed forms and allergy vials?
Allergy vials can be sent to the Student Health Services Pharmacy and should include the required forms from the referring allergist.
Please note: Allergy vials should be properly labeled before sending (patient first and last name, date of birth, contents of vial, dilution, and expiration date). Vials with insufficient labels may be discarded.
