- Medical History (628 Kb PDF) All patients are asked to complete this form at their first visit to the Student Health Center.
When you arrive at the Student Health Center for your appointment, you will be asked to provide information that will assist the provider in recommending the best course of action for your diagnosis.
- Insurance ID card or copy of both front and back of insurance ID card
- Immunization history
- Allergies and reactions
- Medications (medication name, dosage, and strength)
- Vitamins, herbal supplements, and other over-the-counter medications
- Family history of illnesses
All new patients will be asked to complete our Medical History Form. You can download this form and bring it to your appointment.
