The education of your student involves more than just the classes in which he/she is enrolled. It involves learning how to manage time, finances, and yes, even health care.
As a parent, you can ensure that your student is properly equipped to be successful when managing his/her health care. At the initial appointment your student will be asked to provide the following information.
- Insurance ID card or copy of both sides of the insurance ID card
- Immunization history
- Medications the student is currently taking (name of medication, strength, and dosage)
- Vitamins, herbal remedies, and other over-the-counter medicines the student is taking
- Allergies and reactions
- Family history of illnesses
This information will assist your student's provider in properly assessing all symptoms and determining the best course of action in treating your student's diagnosis.
